PERSONAL CARE ASSISTANCE SERVICES (PCA)
Personal care assistance (PCA) services provide assistance and support for persons with disabilities, living independently in the community. This includes the elderly and others with special health care needs. PCA services are provided in the recipient’s home or in the community when normal life activities take him/her outside the home. HOMEMAKER SERVICES (HMK)
Homemaker services provide assistance with general household activities by a trained homemaker when the person (i.e., the person, family member or primary caregiver) who is regularly responsible for these activities is unable to manage the household activities or is temporarily absent. MNSURE APPLICATION ASSISTANCE
Through a contract with MNsure, our certified MNsure Navigators provide hands-on assistance with the application, enrollment, and renewal processes for MNsure and Minnesota Health Care Programs. If you qualify for Medical Assistance or MinnesotaCare, you can enroll in one of these programs through MNsure at any time of the year. Please click here to learn more.
SOCIAL SECURITY DISABILITY BENEFITS/SOAR ADVOCACY
The Social Security Administration offers two benefit programs through which individuals who are disabled and unable to work may receive payments: Supplemental Security Income (SSI) and Social Security Disability Insurance (SSDI). We offer assistance to individuals with the application process, appeals, and benefit maintenance. Our agency contracts with the Minnesota Department of Human Services (DHS) to supply these services at no fee for individuals receiving specific Minnesota state aid. Additionally through our contract with DHS, we provide SSI/SSDI Outreach, Access, and Recovery (SOAR) advocacy. SOAR is a program designed to increase access to SSI/SSDI for eligible adults who are experiencing homelessness, or are at risk of homelessness, and have a mental illness, medical impairment, and/or a co-occurring substance use disorder. HOUSING RESOURCES
We provide information and education about affordable housing options in Minnesota, such as the Section 8 subsidized and voucher program. This includes assisting individuals with the housing search process, guiding individuals through affordable housing applications, and providing information about landlord-tenant rights and responsibilities.
